TriWest Healthcare Alliance (TriWest) has expanded its Community Care Network (CCN) Region 4 contract to include the U.S. Pacific territories of Guam, American Samoa and the Northern Mariana Islands. The transition from Patient-Centered Community Care (PC3) to CCN Region 4 began in January 2022 and is expected to be complete by March 31, 2022, or as otherwise directed by the Department of Veterans Affairs (VA).
CCN is a consolidated network of community-based health care professionals available to supplement care delivered through VA medical facilities. Veterans living in these Pacific territories who are currently receiving care under PC3 will continue with their community health care providers under the new CCN contract. TriWest is privileged to support the health care needs of America’s Veterans and is responsible for building and maintaining a network of community health care providers, paying claims and providing excellent customer service.
